Kemik ili\u011fi nakli, ki\u015finin kemik ili\u011finin yetersiz kald\u0131\u011f\u0131 durumlarda yada ki\u015finin y\u00fcksek dozda kemoterapi &#038; radyoterapi sonucunda yani kemik ili\u011finin zarar g\u00f6rd\u00fc\u011f\u00fc durumlarda yap\u0131lan bir i\u015flemdir.<\/p>\n<p> Tedavi i\u00e7in kullan\u0131lan kemik ili\u011fi hasta olan \u015fahs\u0131n tedaviye ba\u015flanmadan \u00f6nce kendisinden, yak\u0131n akrabas\u0131ndan bazen de tamamen yabanc\u0131 birisinden al\u0131n\u0131r. Kemik ili\u011fi hastan\u0131n kendisinden al\u0131nd\u0131\u011f\u0131 durumlarda, k\u00f6k h\u00fccreler, hastal\u0131kl\u0131 h\u00fccrelerden m\u00fcmk\u00fcn oldu\u011funca ar\u0131nd\u0131r\u0131l\u0131r. Elde edilen k\u00f6k h\u00fccreler hastaya bir kateter yard\u0131m\u0131yla verilir. Kana kar\u0131\u015fan k\u00f6k h\u00fccreler kan yoluyla ait oldu\u011fu kemik ili\u011fine giderek orada h\u00fccre \u00fcretimine ba\u015flarlar.<\/p>\n<p> Neden Kordon Kan\u0131?<\/p>\n<p> Kordon kan\u0131 k\u00f6k h\u00fccre transplantasyonunun ger\u00e7ekle\u015febilmesi i\u00e7in kemik ili\u011fi k\u00f6k h\u00fccre transplantasyonuna oranla daha d\u00fc\u015f\u00fck uyum yeterlidir.<\/p>\n<p> Transplantasyonda ba\u015far\u0131n\u0131n maximum olabilmesi i\u00e7in <\/p>\n<p> k\u00f6k h\u00fccre nakillerinde kullan\u0131lan h\u00fccrelerin hastan\u0131n kendi h\u00fccrelerine m\u00fcmk\u00fcn oldu\u011funca \u00e7ok benzemesi gerekmektedir. Her \u015fahs\u0131n h\u00fccrelerinin y\u00fczeyinde \u0091Human Leukocyte-Associated ( HLA )\u0092 antijen ad\u0131 verilen \u00e7e\u015fitli protein setleri vard\u0131r. \u00d6zel bir \u00e7e\u015fit kan testi ile HLA tiplemesi ad\u0131 verilen test ile tan\u0131mlan\u0131r. Verici ile al\u0131c\u0131n\u0131n ( hastan\u0131n ) HLA antijenlerinin birbirine uyumu ne kadar y\u00fcksekse naklin ba\u015far\u0131s\u0131 da o kadar y\u00fcksek olur. Aile i\u00e7i doku uyumu %25 ve \u00fczerinde seyrederken aile d\u0131\u015f\u0131nda bir ki\u015fiden doku uyumu olduk\u00e7a d\u00fc\u015f\u00fckt\u00fcr. \u0096 HLA uyumu i\u00e7in yap\u0131lan \u00f6zel kan testleri ile, HLA antijenlerine bak\u0131l\u0131r ve nakil merkezleri en az 5 antigenin uyumlu olmas\u0131 durumunu arar, HLA uyumu ne kadar y\u00fcksekse bununla ters orant\u0131l\u0131 olarak GVHD olas\u0131l\u0131\u011f\u0131 d\u00fc\u015fer.<\/p>\n<p> Yeti\u015fkin kemik ili\u011finden k\u00f6k h\u00fccre elde edilmesi i\u00e7in cerrahi m\u00fcdahale ve genel anestezi gerekmektedir. Kordon kan\u0131ndan k\u00f6k h\u00fccre toplanmas\u0131 ise a\u011fr\u0131s\u0131z ve birka\u00e7 dakikal\u0131k basit bir i\u015flemdir, ayr\u0131ca gerek bebek gerekse anne i\u00e7in hi\u00e7bir risk ta\u015f\u0131mamaktad\u0131r ve kemik ili\u011fi nakline g\u00f6re daha kolay ve ucuzdur. Dolay\u0131s\u0131yla g\u00fcn\u00fcm\u00fczde anne ve babalar\u0131n bir \u00e7o\u011fu yeni do\u011fan bebeklerinin kordon kanlar\u0131n\u0131n saklanmas\u0131n\u0131 istemektedirler.*<\/p>\n<p> Kordon kan\u0131ndan elde edilen k\u00f6k h\u00fccre, d\u0131\u015f ortamdan herhangi bir zarar g\u00f6rmedi\u011fi i\u00e7in, \u00fcremeye haz\u0131r durumdad\u0131r. Halbuki kemik ili\u011fi nakillerinde GVHD en s\u0131k rastlanan ve \u00f6l\u00fcmc\u00fcl olabilen yan etkilerden biridir. Nakledilen doku, al\u0131c\u0131n\u0131n v\u00fccudunu yabanc\u0131 doku olarak g\u00f6r\u00fcr ve reddeder. GVHD, 2 y\u0131l i\u00e7inde hastalar\u0131n % 50 sinde ortaya \u00e7\u0131kmaktad\u0131r. Kordon kan\u0131 k\u00f6k h\u00fccre nakillerinde ise kordon kan\u0131 h\u00fccrelerinin antijenik yap\u0131s\u0131n\u0131n hen\u00fcz tam geli\u015fmemi\u015f olmas\u0131 nedeniyle bu oran \u00e7ok daha d\u00fc\u015f\u00fckt\u00fcr.<\/p>\n<p> Bu h\u00fccrelerden en \u00e7ok kullan\u0131lan\u0131 hematopoietik k\u00f6k h\u00fccrelerdir. Hematopoietik k\u00f6k h\u00fccreler; kemik ili\u011fi ve \u00e7evre kan\u0131n\u0131n h\u00fccresel elemanlar\u0131n\u0131 olu\u015fturur.<\/p>\n<p> Kordon kan\u0131 saklanan bebek ilerde k\u00f6k h\u00fccre naklini gerektirecek bir hastal\u0131\u011fa yakaland\u0131\u011f\u0131 durumda uygun bir verici aramaya gerek kalmadan kendine ait k\u00f6k h\u00fccrelerle \u00e7ok daha kolay tedavi edilebilecektir. ( Kemik ili\u011finden k\u00f6k h\u00fccrenin eldesiyle ba\u015fta kanser t\u00fcrleri olmak \u00fczere bir \u00e7ok hastal\u0131k tedavi edilebilmektedir fakat hastalar\u0131n %70\u0092ine uygun kemik ili\u011fi bulunamamaktad\u0131r. Oysa plasentadaki kan bebe\u011fin kendinin oldu\u011fu i\u00e7in bu durum tamamen ortadan kalkm\u0131\u015ft\u0131r.)
